In 2012, Knight Capital Group, a financial services firm, experienced a technical glitch caused by a deployment error in their trading software.

The error resulted in a series of unintended trades and led to a loss of over $440 million in just 45 minutes, almost causing the company to go bankrupt.

TSB Bank — £330 million ($455 million)

In 2018, the British bank TSB attempted to migrate its customer data to a new IT system. The migration resulted in multiple technical issues, including customers being unable to access their accounts and transactions being duplicated.

The botched migration led to weeks of service outages and financial losses, including a reported cost of around £330 million ($455 million).
